The details of the treatment/therapy have been clearly explained to the patient and the patient is willing to abide by the instructions given in this regard. The patient will be responsible for any consequences which may arise from disobeying the instructions of the concerned Physiotherapy Practitioner
The patient has not concealed important information such as present medications, health conditions and has informed the practitioner about his/her known allergies
None of the patient’s family members would be present in the room during the Physiotherapy procedure. If the person is unable to move/very old, one attendant of the same gender is allowed
In case the patient is under 18 years, one parent/ guardian of the same gender will be present throughout the procedure
Patient or patient party is expected to inform the Doctor about the known allergies, tumors, any kind of implants and any previous surgeriesThe description about therapies is solely for educational purpose. Doctor consultation prior taking each therapy is recommended
The patient will not threaten/ harass the Physiotherapy practitioners or the staff in any manner as any kind of harassment would be subject to legal action. The patient will not hold the Ayursh firm or the practitioners responsible for the consequences arising due to the patient’s decision to undergo the treatment
Although we try to allocate same therapist for each treatment session during the multi session bookings, we dont guarantee same
Payment once made will not be refunded
Space for keeping massage table should be allocated in patients home during multi session bookings
During ongoing therapy package, time slot flexibilities is expected from the patient/customer side
Multi session therapy bookings have an expiry period of 90 days from the time of booking. This is to ensure that treatment is done right in time to fetch the maximum benefit out of it.
